Due to the nature of this orthosis and the required intimacy of fit to benefit the patient we request you provide a modified positive cast and a completed orthometry form with all necessary measurements. Additionally we ask for markings on bony prominences and location and size of ulceration. We recommend supplying a foam foot box for foot impression.
